Dog Training Liability Waiver & Release of Responsibility

By enrolling in and participating in any training program, private session, group class, boarding service, or related activity provided by Canyon Lake Hank's K-9 Obedience and Training, you acknowledge and agree to the following terms and conditions.

1. Assumption of Risk I understand that working with dogs involves inherent risks, including but not limited to bites, scratches, falls, property damage, and unpredictable behavior. I voluntarily assume all risks associated with participation in dog training and related services. 2. Health & Behavior Disclosure I certify that my dog is in good health, free from contagious illness, and up to date on required vaccinations. I agree to disclose any known medical conditions, history of aggression, or behavioral issues that may affect my dog’s participation. 3. Owner Responsibility I understand that I am fully responsible for my dog’s behavior and actions at all times. I agree to maintain control of my dog, follow trainer instructions, and comply with all facility rules and safety guidelines. 4. Release of Liability I hereby release, waive, and hold harmless [Business Name], its owners, employees, trainers, contractors, and agents from any and all claims, demands, injuries, damages, losses, or liabilities arising from participation in services, whether caused by negligence or otherwise. 5. Indemnification I agree to indemnify and defend [Business Name] against any claims, lawsuits, damages, or expenses resulting from my dog’s actions, including injury to persons, animals, or property. 6. Property Damage I accept financial responsibility for any damage caused by my dog to facilities, equipment, furnishings, or other property during training, boarding, or related activities. 7. Training Results Disclaimer I understand that dog training results vary based on many factors, including consistency and owner involvement. No guarantees are made regarding specific behavioral outcomes or performance. 8. Photo & Video Release (Optional) I grant permission for [Business Name] to use photographs and videos taken during training sessions for marketing and promotional purposes, unless I submit a written request to opt out. 9. Medical Emergency Authorization In the event of a medical emergency, I authorize [Business Name] to seek veterinary treatment for my dog at my expense when reasonable attempts to contact me have been unsuccessful. 10. Agreement & Acknowledgment By signing below and/or submitting this form electronically, I confirm that I have read, understood, and agree to all terms of this Liability Waiver and Release.
